Where to get a Driving Record

You can order your Delaware driving record multiple ways. Traditionally, you must fill out a state Personal Information Release Form, which must be signed and notarized. Send the form, along with payment, to the following address:

Division of Motor Vehicles
ATTN: Driver License Administration
P.O. Box 698
Dover, DE 19903

Your three-year driving record will then be mailed to you. You can also visit the Delaware DMV in person to request a copy of your driving history, or you can purchase the report through a third party online.
